A health history provides context for everything that follows. Without context, a finding can be misleading. A heart rate of 110/min means something different in a patient who has just climbed stairs than in a resting patient who is pale, weak, and reporting chest discomfort.

8. Pain Is More Than a Number

Pain assessment may include location, intensity, quality, timing, duration, aggravating and relieving factors, associated symptoms, and functional impact.

One useful beginner principle is: the number is only one part of the story. A patient reporting “7/10” pain deserves further assessment of what that pain means, where it is located, what caused it, how it behaves, and what effect it has on sleep, movement, breathing, eating, mood, or daily activities.

9. Neurological Assessment 🧠

Neurological assessment can range from basic observation to highly focused examinations depending on the clinical situation.

Elimination provides valuable information about hydration, renal function, medications, mobility, continence, infection risk, gastrointestinal function, and the patient's ability to manage daily activities.

Assessment may include frequency, urgency, pain, continence, urine characteristics, bowel frequency, stool pattern, constipation, diarrhea, devices, and changes from baseline.

14. Skin and Pressure-Injury Risk

Skin assessment should be integrated into routine care rather than treated as an isolated event. Observe areas exposed to pressure, moisture, friction, devices, immobility, poor nutrition, or impaired sensation.

Document actual findings rather than assumptions. “Red area” is less useful than describing its exact location, size, appearance, temperature, tenderness, skin integrity, and other clinically relevant characteristics according to local documentation standards.

15. Functional Assessment: What Can the Patient Actually Do?

A patient may have medically stable vital signs yet be unable to safely perform essential daily activities. Functional assessment asks how illness affects real life.

Basic ADLs

Bathing, dressing, toileting, transferring, continence, feeding, and other fundamental self-care activities.

Instrumental ADLs

More complex activities such as medication management, shopping, cooking, finances, transportation, and household tasks.

Functional assessment is particularly important for discharge planning. A patient may understand every instruction but still be unable to climb stairs, prepare food, obtain medication, or safely transfer without assistance.

16. Psychosocial Assessment 💙

People do not experience illness as isolated organs. Anxiety, grief, fear, loneliness, financial pressure, family responsibilities, cultural beliefs, communication barriers, and previous healthcare experiences can influence how a person understands and responds to illness.

For example, telling a patient to “return tomorrow” is not a complete plan if the patient has no transportation. Telling someone to “take the medicine twice daily” is less useful if the patient cannot afford it or cannot understand the instructions.

Comprehensive assessment should not become a collection of stereotypes. Culture can influence communication, health beliefs, family roles, food practices, decision-making, modesty, and expectations of healthcare, but individual patients should never be reduced to cultural assumptions.

The phrase “What medications do you take?” may produce an incomplete answer. Patients may forget names or doses. When appropriate, reviewing medication containers, medication lists, pharmacy information, or the electronic record can help verify the history according to local policy.

For example, consider a patient with increasing respiratory rate, new confusion, cool skin, reduced oxygen saturation, and increasing work of breathing.
